Regional Peer Coordinator


 

Primary Objective

The Regional Peer Coordinator provides exceptional customer services and communication to Positively U STARS (patients) We are looking for someone who is passionate about serving the needs of individuals impacted by HIV and has lived experience in HIV and is a subject manner expert. This is a contractual position.

Essential Functions

  • Must have an understanding and passion for the HIV community.
  • Must provide high-quality, coordinated, strengths-based out of the box services consistently with the Positively U, Inc mission statement for people living with HIV.
  • Must have monthly contact Positively U STARS have empowering and innovative touch call.
  • Must be flexible and have adapt Positively U, Inc out of the box concept.
  • Provide empowering conversation.
  • Must attend weekly Positively U, Inc. empowerment meeting.
  • Must facilitate weekly empowerment calls in rotation with team members.
  • Must encourage a minimum of three new STARS to attend our weekly empowerment call.
  • Provide specialized HIV prevention education, including individualized sessions, group interventions, and educational workshops.
  • Maintain files in a safe a secure location.
  • Keep Positively U, case manager abreast of any situation, occurrences, or outstanding changes in the STAR pertinent information.

  • Proficient in EMR system, preferably ECW
  • Proficient in Microsoft Office suite
  • Ability to communicate professionally (verbally and written)
  • Ability to handle sensitive and confidential information.
  • Ability to present a pleasant, professional demeanor and image
  • Mathematical aptitude

Core Competencies

  • Customer Service: The Regional Peer Coordinator and the team is often the intricate contact to our STAR (patient) has with Positively U and must therefore demonstrate exemplary customer service to ensure a positive patient experience. This includes engaging patients, being receptive and responsive to their needs, and appropriately addressing any patient questions or concerns.
  • Effective Communication: The Regional Peer Coordinator must communicate effectively and respectfully with STAR (patients) via phone, via email, and via EHR system messaging. They should be able to listen effectively and express themselves in a clear and articulate manner.
  • Information Collection & Management: The Regional Peer Coordinator must be able to collect, organize, and monitor data accurately and efficiently. This data includes Protected Health Information (PHI), which must be handled in compliance with the Health Insurance Portability & Accountability Act (HIPAA). All personal information (PHI) must be kept in a locked and secure location.
  • Multi-Tasking & Adaptability: The Regional Peer Coordinator must be able to multi-task and adjust their approach to meet changing demands and stressful situations.
  • Personal & Professional Ethics: The Peer Regional Coordinator must have integrity and honesty in this position. Adherence to professional ethics as delineated by Positively U and federal regulations (e.g., HIPAA) is required.

Education

  • High school diploma or equivalent required.

Experience

  • Must be compliant with your personal medical care and treatment.

Position Type and Expected Hours of Work

This is 30 or 60 hours per month contract. This includes self and assigned training, monthly touch calls, entering notes in EMR E-clinical Works, and contact case manager with any personal changes or outstanding occurrences.

Job Type: Contract

Pay: Up to $18.00 per hour

Education:

  • High school or equivalent (Required)

Experience:

  • Community health center: 1 year (Preferred)
  • HIV/STI: 4 years (Required)

Work Location: Remote
